Cervical Dystonia (Torticollis): Symptoms and Treatment

If you’ve noticed your neck involuntarily twisting to one side, your head tilting without you meaning it to, or a persistent ache in your neck and shoulder that doesn’t respond to regular painkillers or physiotherapy, cervical dystonia may be the underlying cause. It’s one of the most common conditions I treat and one of the most frequently misdiagnosed, often mistaken for a chronic muscle strain or cervical spondylosis before a neurologist is ever consulted.

This guide walks through what cervical dystonia actually is, how to recognize it, and what treatment realistically looks like.

What Is Cervical Dystonia?

Cervical dystonia also called spasmodic torticollis is a neurological condition in which the muscles of the neck contract involuntarily, causing the head to twist, tilt, or pull into abnormal positions. It’s classified as a focal dystonia, meaning it affects one specific part of the body rather than being generalized.

The movements aren’t something the patient can consciously control or fully suppress, which is often the most frustrating part for patients friends and family sometimes assume the posture is voluntary or habitual, when it’s actually a neurological signal misfiring.

Torticollis vs. Cervical Dystonia – Same Thing?

“Torticollis” is an older, more general term describing any abnormal twisted-neck posture, including congenital torticollis in infants (usually a muscular issue, not neurological). Spasmodic torticollis specifically refers to the adult-onset neurological form, this is what most people mean when discussing cervical dystonia in a movement disorder context.

Common Movement Patterns

Cervical dystonia can present in several distinct patterns, sometimes in combination:

  • Torticollis – head rotates/turns to one side
  • Laterocollis – head tilts toward one shoulder
  • Anterocollis – chin pulls downward toward the chest
  • Retrocollis – head pulls backward

Many patients also develop a head tremor alongside the abnormal posture, which can further complicate early diagnosis if a tremor disorder is suspected first.

Symptoms Beyond the Visible Posture

The abnormal neck posture is the most visible sign, but it’s rarely the only symptom:

  • Persistent neck and shoulder pain, often worse by the end of the day
  • Headaches, sometimes migraine-like in intensity
  • A “geste antagoniste” – many patients discover that lightly touching their chin or cheek temporarily reduces the spasm, a distinctive clue that helps confirm the diagnosis
  • Muscle hypertrophy (visible enlargement) on the overactive side, from years of sustained contraction
  • Difficulty with tasks that require holding the head steady – reading, driving, or using a screen for extended periods

What Causes Cervical Dystonia?

In most cases, the exact cause isn’t identifiable, this is called idiopathic cervical dystonia, and it accounts for the majority of the patients I see. What we do know:

  • It involves abnormal signaling in the basal ganglia, the brain region responsible for coordinating muscle movement
  • A genetic component is present in some cases – roughly 10-25% of patients have a family history, and mutations in genes like THAP1 or GNAL have been identified in some families
  • It can occasionally develop secondary to other conditions: prior head or neck trauma, certain psychiatric medications, or as part of a broader movement disorder

In my practice, one of the first things I explain to newly diagnosed patients is that idiopathic cervical dystonia is not something they caused through posture, stress alone, or a specific injury they can pinpoint – this reassurance matters, since many patients arrive having spent months blaming themselves for a “bad habit” they couldn’t seem to break.

How Is It Diagnosed?

There’s no blood test or scan that definitively confirms cervical dystonia – diagnosis is primarily clinical:

  • Detailed history – onset pattern, triggers, family history, medication history
  • Physical examination – observing the specific movement pattern, testing for the geste antagoniste response, checking for a head tremor
  • Ruling out other causes – cervical spine imaging (MRI/X-ray) to exclude structural issues like a herniated disc, and blood tests if a secondary cause is suspected

A significant proportion of patients I see have already gone through months of physiotherapy or orthopedic evaluation for presumed “muscular” neck problems before a neurologist correctly identifies the dystonia.

Treatment Options

1. Botulinum Toxin Injections – the primary treatment

For most patients, botulinum toxin (Botox) injections directly into the overactive neck muscles are the most effective treatment. This isn’t a cure, but it significantly reduces both the abnormal posture and the associated pain for the majority of patients, with effects typically lasting 3-4 months before a repeat session is needed.

2. Oral Medications

Used either alongside Botox or for patients who don’t respond fully to injections options include muscle relaxants and anticholinergic medications, though side effects (dry mouth, cognitive fog, fatigue) often limit their long-term use.

3. Physical and Occupational Therapy

Targeted physiotherapy – distinct from generic neck-strengthening exercises can help maintain range of motion and reduce compensatory muscle strain in surrounding areas.

4. Deep Brain Stimulation (DBS)

For a smaller subset of patients with severe, treatment-resistant cervical dystonia, DBS targeting the globus pallidus can provide substantial relief when Botox and medications aren’t sufficient.

Living With Cervical Dystonia

Cervical dystonia is a chronic condition for most patients, but “chronic” doesn’t mean unmanageable. With consistent Botox therapy and the right supporting treatments, most of my patients maintain a good quality of life and continue working, driving (once symptom control is adequate), and staying socially active.

The biggest improvement I’ve seen in patient outcomes over the years isn’t a single new treatment – it’s earlier diagnosis. Patients who reach a movement disorder specialist within the first year of symptoms typically respond faster and with fewer complications than those who spend years cycling through other treatments first.
